About the Role

Adventist Health Glendale — a 515-bed, Magnet-designated hospital between Burbank and Pasadena — is hiring a Care Manager RN, per diem, day shift.

What You'll Do

• Collaborate on development, implementation, and reporting of the case management program
• Serve as liaison between patient, family, care team, and healthcare benefactor
• Coordinate a continuum of care from pre-admission through post-discharge
• Monitor appropriateness/timeliness of care; discuss resource utilization and discharge plans with physicians
• Coordinate transfers to tertiary centers and collect data for regulatory/accreditation compliance

Requirements

• BSN preferred; care management experience preferred
• Active RN licensure required; case management certification preferred
• BLS certification required
